Harnessing the commitment of staff across the NHS in England to make care safer.
Our vision is for the whole NHS to become the safest healthcare system in the world, aiming to deliver harm free care for every patient every time. This means taking all the activities and programmes that each of our organisations undertake and aligning them with this single common purpose.
Organisation name: Lakeside Healthcare at The New Queen Street and Stanground Surgeries
In signing up, we commit to strengthening our patient safety by:
- Describing the actions (on the following pages) we will undertake in response to the five campaign pledges
- Committing to turn these actions into a Safety Improvement Plan which will show how our organisation intends to save lives and reduce harm for patients over the next three years (acute, ambulance, community, mental health providers only)
- Identify the patient safety improvement areas we will focus on
- Engage our local community, patients and staff to ensure that the focus of our work reflects what is important to our community
- Make public our commitments and plans.
The five Sign up to Safety pledges
1. Putting safety first. Commit to reduce avoidable harm in the NHS by half and make public our locally developed goals and plans. We will continue to improve our performance in identifying potential risk at every level of the organisation by:
- working in an open and honest way and investigating when things go wrong to try to improve our performance
- work closely with our patient participation group and discuss with them openly about patient safety in our organisation and our efforts to improve where possible and be open to any suggestions they may have for us
- Strive to continue to improve on our performance in the areas of:
- Health and Safety
- Significant Event Reporting
- Complaint Handling
- Risk Assessment
- Infection Control
2. Continually learning. Make our organisation more resilient to risks, by acting on the feedback from patients and staff and by constantly measuring and monitoring how safe our services are
- Listen to feedback we get from our patients through Friends and Family test and any other patient surveys we take part in
- Listen to our staff and explore with them any ways we can of improving how we work to lessen any risks they face
- Ensure all our staff are adequately trained in all the mandatory areas to help us collectively prevent risk
3. Being honest. Be transparent with people about our progress to tackle patient safety issues and support staff to be candid with patients and their families if something goes wrong
- Communicate with our patients and the patient participation group openly with annual reports on complaints and significant events and through Friends and Family Test reporting called ‘you said/we did’
- Support our staff with mentoring and or training as appropriate if we identify that they need such support following a patient safety incident.
- Share our practice data around safety and not be afraid to discuss what we faced and what we have implemented to discourage a recurrence in the future.
4. Collaborating. Take a lead role in supporting local collaborative learning, so that improvements are made across all of the local services that patients use
- Instigate a regular agenda item at all ‘Practice Manager Meetings’ called Patient safety so that we can share best practice amongst our locality and learn from each others' experiences.
- Already added a standing agenda item to our Patient Particiaption Group monthly meetings called PATIENT SAFETY - so that I have an opportunity to discuss any issues with them and gather feedback from them
5. Being supportive. Help our people understand why things go wrong and how to put them right. Give them the time and support to improve and celebrate progress
- Involve any team member named in a complaint so that they can take part in forming the response and then assist the practice to ‘own’ any changes or learning needed as result of this process
- Involve any team member named in a significant event so that they can assist in forming our collective response to this process.
- Support staff with training identified for them to enable to them to undertake their job role to the best of their abilities.
- Not ask our staff to perform any duty for the organisation that they are not appropriately trained or supported to do.
